This membership application is used by all new, re-enrolled and charter members. It is designed as a tri-fold brochure and you need to print page one and then turn your paper over and print page two.
The written application portion is to be filled out on both sides, removed and given to the Secretary. The remaining information and receipt portion is given to the applicant.
